Optimising person-centered care and self-management for patients in the community

Nurses are central to reducing hospital attendances and admissions by optimising person-centred care and self-management for patients across all settings within the UK.
This programme will be led by the District and Community Nursing Forum with three senior NHS nurses sharing their evaluation, learning and successes from recent NHS commissioned models of self-care.
This session aims to:
- support understanding of the principles underpinning person-centred care and self-management of patients relevant to health care policy in each country
- encourage reflection on the importance person-centred care and self-management of patients, including the benefits and challenges
- facilitate new ways of thinking and innovation centred on partnership working with patients.
This session is relevant to all health care staff, including student nurses.
