The wrong age for care? Transition and provision

Delegates will hear from a young person living with a life limiting disease about their experiences and what makes a difference to them, followed by an interactive session discussing how to support young people living with a life limiting disease through transitions. They will also hear about the tools and services that can make a difference to their practice and the experience of young people and their families.
